  1. Hospitals are separated into three groups: Intervention: Group 1 (Early intervention); Early control/later intervention: Group 2; and additional hospitals not included in research design: Other. The first 4 variables in this table were those used to systematically assign the hospitals to Group 1 or 2. The “Other” hospitals participated in selected meetings and trainings, but were not included in the study design.
